The California Department of Mental Health (CDMH) is continuing its evaluation system for Children’s Mental Health Services (CMHS). The collection of measures will occur twice a year, during two-week periods in May and November. Information will be collected from parents/caregivers and youth (13 yrs and older) on satisfaction with services and service impact.

The assessment tools, which are the Youth Services Survey for Youth (YSS-Y) and the Youth Services Survey for Families (YSS-F), were chosen by the state and are to be completed on every child that receives services during the data collection period. All mental health services are included except for inpatient services, juvenile hall services, juvenile justice camp services and crisis services. All of the items are to be completed directly by the parent/caregiver or youth.

The next YSS data collection is scheduled for November 3-17, 2008.

Quick Reference:

- Administered to all parents/caregivers (YSS-F in the White cover sheet for English and Goldenrod cover sheet for Spanish) regardless of the youth's age AND youth 13 years and older (YSS in the Blue cover sheet for English and Purple cover sheet for spanish) who receive billable services during the YSS period.

- If you need additional forms: Contact our office and allow 24 hours to receive additional materials. Please DO NOT make photocopies.

- Fill in the program information BEFORE giving the packet to the youth/parent on the bottom of the form pages as well as the envelope: 1) CSI County Client Number (Client's ID number - same number used in INSYST/Anasazi), 2) Subunit, 3) Date survey was administered and 4) Reason not completed (if applicable).

- Please make sure respondents have PRIVACY when filling out forms and upheld at the utmost importance and confidentiality. Surveys should be returned to the program in the sealed envelope and not be opened by staff.

-You can return forms at anytime: 1) in person at the SOCE office or 2) through the interoffice mail with Rady Children's Hospital. Forms can also be picked up by a SOCE member during the designated dates.

  • YSS Information Distribution: October
  • YSS Administration Training: Thursday, October 30th at 10-11am at SOCE office
  • YSS Data Collection: November 3 - 17, 2008
  • Submission Deadline: Friday, November 21st at NOON
    You can return forms at anytime 1) in person at the SOCE office 2) through the interoffice mail with Rady Children's Hospital 3) or at a drop off site on the designated dates (drop off sites TBA).
